We strive to make every visit smooth, convenient, and stress-free for your family. Please review the information below before your appointment.

Scheduling & Cancellations

Please schedule appointments as far in advance as possible. We kindly request at least 24 hours' notice if you need to cancel or reschedule. This allows us to offer the appointment to another family in need. Late cancellations and missed appointments are subject to a cancellation fee.

Parent & Guardian Information

A parent or legal guardian must accompany patients under 18 years of age. If another trusted adult is bringing your child, please provide written authorization. Legal guardians should also bring guardianship documents so they can be added to your child's medical record.

Vaccinations

We are vaccine neutral and generally follow the CDC immunization schedule while respecting parents who choose an alternative schedule.

Advance Notice Required
Please notify our office at least one week before your appointment if your child will be receiving vaccines, as some vaccines may not always be in stock.

Common vaccines requiring advance notice:

  • Pneumococcal
  • Influenza (Flu)
  • MMR
  • Varicella (Chickenpox)
  • COVID-19
All routine vaccines are also available free of charge through the Maricopa County Health Department Immunization Clinics.

Phone Calls

Our team answers calls Monday – Friday, 8:00 AM – 3:00 PM.

If you reach voicemail during office hours, we are assisting another family or caring for a patient. Please leave one message and we will return your call as soon as possible.

Calling multiple times in succession delays our ability to assist everyone efficiently. We appreciate your patience while we return messages in the order received.

After-Hours Care

For routine questions, appointment requests, or minor illnesses, please leave a message on our after-hours voicemail. Your call will be returned the next business day.

Established patients experiencing an urgent medical concern that cannot wait until the next business day may contact the on-call physician by following the instructions provided on the after-hours voicemail.

Prescription Refills

New medications, including antibiotics, require an appointment with the patient before they can be prescribed.

For chronic conditions, we are happy to assist with prescription refills.

Billing & Payment

Insurance plans may require co-pays, co-insurance, or deductibles. These payments are expected at the time of service.

Please ensure whoever accompanies your child brings the required payment with them.

Insurance Responsibility
Families are responsible for understanding their insurance coverage, provider participation, deductibles, and non-covered services.

Professional Fees & Cash Pay

Our fees are reviewed regularly and are based on the complexity of care, including the severity of illness, tests ordered, medical records reviewed, examination findings, time spent with the patient and family, and counseling provided.

Cash pay pricing and additional payment information are available through our office.

We are committed to protecting your child's privacy while maintaining a safe, respectful, and supportive environment for every family.

Medical Records

Requests for medical records must be submitted in writing and include the legal guardian's signature authorizing the release of information.

Patients who are 18 years or older must personally request their own medical records.

Records sent directly to another physician are provided at no charge.

Fees may apply for records copied for insurance reviews, determining insurability, or personal use.

Disruptive behavior, personal attacks, harassment, or repeated non-constructive criticism may result in dismissal from the practice.
